FAYS Family Support Specialist - Family & Youth Success (FAYS) Program

BASIC FUNCTION:
Assesses needs and provides counseling for clients and families qualifying for FAYS program; secures emergency placement as needed. Manages, implements, and coordinates the case management. Duties include gathering information, identifying services, writing comprehensive service plans, and monitoring the delivery of services to the citizens of Deep East Texas.

DUTIES AND RESPONSIBILITES:
The below statements are intended to describe the general nature and scope of work being performed by this position. This is not a complete listing of all responsibilities, duties and/or skills required.
−Conduct assessments to determine family needs and develop individualized service plans.
−Develops a comprehensive plan of service for case management.
−Maintain accurate and up-to-date case files and documentation.
−Provides crisis intervention when necessary.
−Provides individual, group, and family counseling to effect behavior change.
−Monitors and reviews Service Plan.
−Conduct home visits and attend community events to engage with families.
−Plans and prepares discharge summary.
−Identifies and evaluates problems, needs, and resources relating to the development of services for the clients.
−Has contact with clients, volunteers, program specialists, local, state, and federal agencies, elected officials, private contractors, and the public.

EDUCATION AND EXPERIENCE:
Bachelor's degree in behavioral science, social service, or a related field, specialized and/or on-the-job training in social casework including crisis intervention, individual and group counseling; plus at least one year of relevant experience working with youth. Bilingual Spanish a plus.

TRAVEL REQUIREMENTS:
-This position requires frequent travel within the [City/Region/State] area to meet with families, attend community events, and providers.
-Occasional travel outside the primary service area may be required for training or conferences.
-Reliable transportation and a valid driver's license are required.
-Travel expenses will be reimbursed according to company policy.

ADDITIONAL REQUIREMENTS, PERSONAL SKILLS AND ATTRIBUTES:
- Must have a VALID Texas driver’s license, proper vehicle insurance and use of vehicle to travel throughout the region
- Utmost professional and personal integrity
- Personal values consistent with the Deep East Texas Council of Governments.
SALARY:
$38,000-$45,000 per year depending on experience, plus benefits including health insurance, retirement plan and other benefits

Salaries of the Deep East Texas Council of Governments are funded by various state and federal grants; therefore, employment is conditioned on the availability of such funds.

FAYS Case Managers provide one-on-one coaching or counseling and group-based learning for youth and parents.
